Does Coughing or Sneezing Increase Hip Impingement Pain?

How Common Is Hip Impingement?
Femoroacetabular Impingement can happen in many different sports and athletes of different genders. Normally it is found in higher correlation with athletic populations in predominately young, white athletes with normal basal metabolic index (BMI). We use to believe it was a condition men would only get but women are not immune from it by any means.1 This is probably because girls take part in more youth sports than now.
CAM deformities have been found to develop in highly active young athletes, more so than non-athletic ones.
The development of a CAM deformity can be correlated with high activity levels where the growth plate of the femur has not totally closed. Typically, athletes will experience a lack of abduction, flexion and rotation from the CAM.
What sports are safe for these CAM athletes?
Logic would lead us to believe that the more we require the athlete to be forced into the pinched motion, the more damage to the joint. This can lead to labral tears and femoroacetabular impingement. The CAM deformity decreases the athlete’s ability to move the hip into flexion, adduction, and internal rotation. Look at the motions required by the sport and see if these are needed to high degrees.
Some sports you should be aware of are:
Golf
Baseball
Tennis
Lacrosse
Gymnastics
Ballet, and other types of dance
Hip Pain Baseball
In the past, many clinicians have felt femoroacetabular impingement was idiopathic, or that it arose spontaneously.
Yet studies have continually shown a higher association in athletic populations than in non-athletic populations, which leads us to believe that there are other correlating factors. One study noted around 55% of all patients are female with the typical surgical age of 28. 2,3
A recent systematic review found that there is a higher correlation of CAM deformity in athletes participating in high impact sports (i.e. basketball, hockey and soccer) over non-athletes. 3
A radiology study showed a correlation of CAM deformity development in youth soccer players whose growth plates are still open.
High prevalence of CAM deformities were found via x-ray in these kids even after initial x-rays showed no signs of CAM development two years earlier.
Overhead athletes are especially high risk due to the need of larger energy motions of the hip.
This demand on the hip can lead to labral tears and increase symptoms of CAM deformity.
Logic says our young athletes involved in club sports and year round programs will have a higher potential of CAM deformities than those who take breaks, or change sports.

With Josh Beckett being a pitcher, there is a large need for a normal functioning hip with normal range of motion, strength, and power.
While analyzing Beckett’s outings where he complained of hip pain, his reports of pain were commonly made during the later innings when his command and control decreased. More pitches went up in the zone to the glove side.
These findings are not uncommon for pitchers that use less legs/ hips in their delivery4. They are depending upon more of the upper body for their velocity. Because of this, pitchers unable to use their legs/hips can, and probably will, develop shoulder injuries, such as rotator cuff tears and SLAP tears.
Do not continue to pitch without knowing how to use your lower body.
You will only experience low velocity, less command, more frustration and more shoulder pain as time goes on. Shoulder injuries in pitchers typically develop from lack of proper deceleration.
Femoroacetabular impingement in pitchers can ultimately lead to lower stride length, and poor command of pitches leaving them high to the glove side.

    Hello, I'm glad to hear the information that you provided in this video. I've been suffering from pain in these areas for about 3-4 years now. I have tried to resolve my pain by visiting different physiotherapists in my city, but with no success. I initially had a diagnosis of CAM-type FAI, but what bothered me the most and still bothers me is a strong tension in the anterior wall of the abdomen very close to the pubic region. I've also had periods of feeling some twinges in the infra-pubic region. In addition, my adductors always seem to be tense along with my pelvic floor. And all of this worsens when I spend long periods sitting or doing tasks that require a lot of hip flexion. What caught my attention the most was indeed the idea of avoiding 'Hollowing' - because curiously I feel more comfortable when I project the tension to the external walls of my abdomen, as if I were relieving all that tension and compression that I feel on a daily basis. It's a pity that when we're in 'automatic' mode, the system adjusts to positions that generate pain and discomfort. I hope one day I could find a therapist who could help me "solve" or at least manage my pain. Until then, I'll manage on my own - searching for information like what you shared in this video.

    I hesitate to suggest deviation to what's presented with these things. I wouldn't just do one side but both with just about anything one does. At least give half as much time to the opposing side. And, with respect to breathing...Yes, originate breath in the lower abdomen however, it's important to fill the whole cavity. The body is whole unit and many have issues with tension/restricton in the upper portions of the chest as well. Think of it like "surrendering into position" ebb and flow originating lower to upper like a wave expansive in all directions. Play with it anyway and see if that provides relief for you. As always, important to discuss these things with competent health care professionals.

      Thanks for sharing. These are short term options. While we know the body adapts to chronic loads and exposures, the idea is to decrease pressure on a specific structure as a way to decrease pain. Less pain opens up more exercise and movement options to allow for bilateral chronic load exposures for a better functioning human

    Can this cause a lump? I was squatting with a barbell ego lifting a bit. A couple days later i woke up to a horrible cramp in my upper abdomen. In the morning i had a slightly painful lump in an inch or two above my belt in line with my left femur, and tenderness in my left teste. Also had some pain at the top of leg when i would walk or move. My flank on that side closer to my back is slightly enlarged. The pain was never severe and lasted only a few days. Occasionally if i exert myself i get some mild pain and tenderness in my left side. Doctor said my ct looked good and referred me to a gastroenterologist. It seems logical to me this is a hernia, or a torn muscle or something to me. Would something like this be obvious on a CT scan?

      Depends on the case. People who don’t tolerate compression forces on the spine should not be hard bracing as it increases the force. They should keep pressure but not brace. Classic with stenosis cases

      @@Performance-Place-chiropractor im not so sure now.. after i did the pinch roll it seems to alleviate the issue.. but last night after the pinching. I decided to use my massager gun ( bob and brads brand). And went over the same places i did the pinch rolls.. the next day.. today. We visited a museum all day.. i was okay with my knee reminding me it was still an issue.. but the end of the day. 10 am to 4 pm. I realized now how so stiff my knee was.. the worst is ever been.. i think its the fact that i ve been in the standing mode for too long ( which is new to me). Im icing it now and have taken an iboprofen.. i m very scared for my future.. i thought i was finally getting out of this deep funk.. but.. i think i may have more of a serious thing going on. Any suggestions please???

      hello- Last night i discovered you.. and got so excited.. today we went to a museum and walked around most part of the day 10-4 and at the end of the day my knee became so stiff.. the stiffest its ever been. This was a big let down for me considering how great i felt yesterday with your method. I m thinking my guess that i have this pesky answering bursitis may not be the correct diagnosis?? No dr told me i had this.. i guessed it based on the location of pain. Mamma Mia! Any thoughts please ?

    I have that exact area of pain in the butt area but it goes to the front of my groin area and a bit down the front of my thigh. I’ve been through 3 years of hell even having FAI labrum repair surgery. Now MRI shows my labrum is torn again. I’ve had PT, hip joint injections, psaos injection..nothing has helped. Now I’m being told I need my pelvic floor “therapied” I’m so frustrated 😢

    Fascinating stuff but my pain remains a total mystery. Like a dull tooth ache,sometimes more sometimes less never very sharp just a dull persistent throbbing like pain and seems unaffected by movement or no movement. From your descriptiopn it sounds most like a groin hernia (which is not a hernia...) hope it isnt a real hernia doesnt seem to be because cant find anything sticking out anywhere! It is a sort of unpleasant toothache deep in the groin area on the right side. Im reasonably but not extremely atheltic, which makes it more difficult to say if it is a groin hernia or not. If I find anything out good or bad which might interest or help people here, I shall write another message. I have an appointment with a physiotherapist and if I do not get enlightenemnt there, I shall have to get my groing examined by a doctor I suppose. I have a strong feeling if I lived in the USA and wandered into your practise you could probably tell me immedately what Ive got. You sound like a man who knows his onions.

    I live in Thailand now and I'm in a bar waiting for a boat back to the island i live on. I came to this island for the same injection that i had in my country (England) when i had a flare up before. I haven't had one for several years but i slipped on wet concrete steps a few months ago and have been in trouble since then. I literally just had it so I can't say it's worked yet but it always did before. I''ve come to this video for tips on self maintenence and i do believe in that but in the comments there's people saying their situation is unbearable. For people unlucky enough to be in this situation the the injection is a sub ilio lumbar injection consisting of 40mg Methylprednisolone and 10mg Bupivacaine. Trust me, I tried everything except surgery but nothing ever helped me until this. If this cooment gives even one person the relief it gave me I will be delighted...

    I got a BS diagnosis of FAI. I got tests done on my hips and they were fine, so doctors couldn’t figure it out. I probably have damage to an SIJ ligament or mobility issues in my lower back. McKenzie extension doesn’t help either. Kind of screwed.
